University staff, students and contractors occasionally need access to facilities data in the course of business or coursework. The Emergency Management Department reviews requests for information about university facilities and grounds to ensure it can be provided without compromising the safety and security of our campus.

To request the information, please complete one of the below electronic forms by selecting the option that best describes you. You may also request an emailed copy by contacting Emergency Management.
University Student/Employee Request

Instructors should make requests on behalf of their students. The instructor should complete the form and attach a list of the names of the students who require the information.
After review, the requestor will be notified of approval via email by the Emergency Management Department. The email approval will include document pick up information. You will need your University ID card to retrieve the information you are requesting.
